PLEASE BE ADVISED THAT BECAUSE WE HAVE REACHED IN EXCESS OF 10,000 PATIENTS REGISTERED AT SOUTHGATE MEDICAL GROUP OUR PATIENT REGISTRATIONS ARE NOW CAPPED WITH EFFECT FROM MONDAY 18TH APRIL 2016

THIS MEANS THAT WE ARE UNABLE TO TAKE ON ANY NEW PATIENTS AS OUR LIST HAS BEEN CAPPED FOR THE DUTY OF CARE TO ALL OF OUR EXISTING PATIENTS AND FOR THE SAFETY OF OUR CLINICIANS

IF THIS SITUATION CHANGES IN THE FUTURE, WE WILL PLACE DETAILS HERE & ON OUR WEBSITE AND IF SO, THEN THE RULES FOR REGISTERING AT SMG WILL REVERT BACK TO THOSE BELOW:

You can register as a patient at Southgate Medical Group ONLY if you live in Southgate. We no longer accept patients from any other part of Crawley and we continue to not accept patients from outside of the town.

Please register your baby as soon as possible after their birth. The doctor is unable to do their 6 - 8 week check if the baby is not registered which makes the immunisation programme delayed as the check needs to be done first

As part of the normal registration process you will be registered as a patient of the practice. You do, however, have the opportunity to state a preference to receive services from a specific practitioner. Where this request is made we will endeavour to comply with your request.

Home Visits

If you are too ill to attend the surgery, please telephone the surgery as early in the day as you can. The receptionist will ask you to provide some details about the nature of your call and the doctor may contact you for an assessment before visiting.

Patient Responsibilities

To help the practice provide the best possible service to our patients:

  • Notify us if you cannot attend an appointment.

  • Arrive promptly for your appointment.

  • Notify us if you change your address or telephone number particularly mobile numbers

  • Show consideration to other patients and staff at the surgery.

  • Give your health care provider as much information as possible.

  • Follow the prescribed and agreed treatment plan.

Violent Patients

It is the aim of Southgate Medical Group to provide a safe and pleasant environment in which patients and visitors may receive healthcare and staff may carry out their work.  A code of conduct is displayed in our surgery outlining how we expect patients to behave when attending or telephoning the surgery. Any person acting in an unacceptable manner will be dealt with in accordance with the code.

Physical or verbal violent behaviour will not be tolerated and will result in police prosecution alongside removal from the practice list.
